Understand you are being recorded & supervised remotely with audio and visual for safety and insurance purposes. Studio is under 24/7 surveillance.

ABSOLUTELY NO ALCOHOL or SMOKING OF ANY KIND ALLOWED. If staff even SUSPECTS, event will be shut down with forfeit of rental booking at full price.

No confetti allowed. No balloons with confetti inside. BOUNCY HOUSES ARE ADDITIONAL CHARGE $25/HR

No drilling or any holes allowed. Use of tape or command strips allowed as long as it does not damage walls, floors, or ceilings.

All decorations, including adhesives must be taken off before rental ends.

***We have neighbors. Respect our neighbors. Absolutely no hanging out or noise outside of studio premises at all times. Especially during entering and exiting we ask that guests keep low noise. No shouting or screaming outside our gate. All guests should remain inside studio during event to avoid disturbance or possible shut-down of events by local precinct.

Renters must be present at all times during their rental booking. Your guests may not be in premises without the presence of renter submitting this form.

Only rental times paid for are authorized time slots booked. You are to remain in use of studio premises during paid slots ONLY. This includes putting things back, gathering belongings, etc.

All studio house lights must be turned on at least 20 minutes before rental end time to begin the clearing out process.

Renter and all guests must vacate the premises (including lobby area) within rental end time to respect time paid for. The only exception to this is if you’re locking up studio you will be authorized an additional 10 minutes to shut down and lock up the studio entrance gate.

All renters are time-tracked and will not be considered for future rentals if they cannot adhere to strict guidelines. If you need more time, please book more time.

It is the renter's responsibility to consider time needed to book for set up and clear out of all guests including yourself. If you are locking up, an additional 10 minutes is automatically given to ensure closing instructions are met. All guests must exit the premises within rental end time, otherwise you will be automatically charged at the rate given in 30 min. intervals. Example, if booking time was 3pm to 6pm and closed gate by 6:18, you are automatically charged for an extra 30 min. No exceptions.

Due to high demand full payments must be made to confirm bookings.

Extensions may be requested AT LEAST 1 hour prior to rental end time & is subject to approval which is determined by availability and cooperation.

Renters are fully responsible for studio premises, equipment used, and all guests during rental.

Large trash cans are provided for your convenience. Please make sure trash goes into receptacles. This message must be relayed to your guests to ensure it is enforced.

We allow renters to use our Bluetooth adapter with charger for sound system if needed. We ask that this and any other equipment/devices be left in their respective area.

ANY items, furniture or equipment cannot be moved or used without authorization prior to booking. If authorization has been given by Jessica, it must be put back EXACTLY as was (we recommend taking pictures before moving items to refer back to). This message must be relayed to your guests to ensure it is enforced.

BENCHES are for SITTING ONLY. Please make sure there is no standing or laying down on benches. Avoid placing shoes on benches. This message must be relayed to your guests to ensure it is enforced.

If you have been authorized to use chairs, they must be placed exactly as was. We recommend taking a picture prior to moving any furniture.

Absolutely no light switches, electric switches, and/or device switches are to be touched without authorization by renters or guests of renter. This message must be relayed to your guests to ensure it is enforced.

No one is allowed in partitioned areas.

The sound system is ONLY to be operated by renter. It is not to be touched or modified in ANY way by guests. This message must be relayed to your guests to ensure it is enforced.

If sound equipment does not work properly, contact Jessica immediately! Do not attempt to fix the problem yourself. We ask everyone to adhere to the same protocols for using sound system to ensure no issues occur during the next rental.

Note, the use of ceiling party lights is an additional rental fee. If used without permission, you will be charged. You may request if you’d like to use them.

If you have opened studio, lock is to be left in designated area by house lights during booking to ensure it is visible and accessible at all times during & after studio use.

Studio space must be cleared of personal belongings in the allotted rental time.

Please check that no personal belongings have been left behind. ELLAVATIONS, INC. DBA BALMIR DANCE STUDIO is not responsible for any personal belongings left or stolen on the premises.

Renters are responsible for any damages that may occur during the rental time. If ANY damages occur, Jessica MUST be contacted immediately. Renters may be subject to fines or charges. Failure to get in contact with Jessica, may cause charges to increase due to neglect.

Management reserves the right to remove anyone who violates this agreement and terminate the use of the premises at any time.

This link with policy and instructions is always available to refer back to if ever needed.

Bouncy houses are an additional $25/hr and must be booked in advance with set time.

Due to COVID Most Renters Will Be Responsible For Opening & Locking Up Studio

SEPERATE INSTRUCTIONS FOR OPENING STUDIO AND LOCKING UP *

I understand it is my responsibility to open studio under the "Opening Studio Instructions & Guidelines"

I understand I am not allowed to share safe box code or allow others to open studio. Failure to follow this will result in loss of future rental privileges.

I understand it is my responsibility to close (lock up) the studio under the "Locking Up Studio Instructions & Guidelines"

CANCELLATION POLICY & FEES *

I understand there are absolutely no refunds, only reschedules when studio is available. No exceptions.

I understand I have only 7 days prior to my booking to reschedule without any penalty. Rescheduled date must be within 30 days from initial booked date.

I understand rescheduling less than 7 days in advance, but not on the same day, will result in 50% of total booking amount charged.

I understand same day cancellations result in being charged 100% for total booking.

OVER-TIME CHARGES *

I understand that any additional time requested the "day of event" will result in an additional $50/hr PLUS hourly rate. Make sure to book hours in advance or lock-up on time to avoid additional charges.
